Electronic Document Delivery Form

GENERAL AGREEMENT – For the purpose of Authorization the term “LRAA” refers to the Lost River Airport Tracts Association.

ELECTRONIC DOCUMENT DELIVERY – As a Member of LRAA, you hereby request and consent to LRAA sending notices, including but not limited to the Annual Meeting Notice, and other communications in Portable Document Format (PDF) to the e-mail address listed below. This Electronic Document Delivery service replaces the creation of paper notices and other communications and their delivery through land-based mail services.

In the event of equipment failure and you are unable to retrieve your email; you may obtain a copy of any notice or other communication on paper by sending a written request to: Lost River Airport Association, PO Box 1272. Winthrop, WA 98862.

YOUR RESPONSIBILITIES – Equipment Access requirements: In order to receive Electronic Document Delivery, you must:
• Have an active email account and access to a device with access to the internet.
• Adobe Acrobat reader 6.0 or higher (free software that you can download)
• Immediately notify LRAA of any change in your email address by sending a letter addressed to:

Lost River Airport Tracts Association, PO Box 1272. Winthrop, WA 98862, or by using the Contact Form provided on the LRAA website. 

WARRANT – You represent and warrant that you have the equipment and access capabilities to receive documents electronically.

CANCELLATION – You may cancel the Authorization and request paper notice and other communication delivery at any time by providing a thirty (30) day written notice by sending a letter addressed to: Lost River Airport Tracts Association, PO Box 1272. Winthrop, WA 98862.

OTHER CANCELLATION RIGHTS – LRAA may also cancel this Authorization and opt to deliver notices and other communications by paper at its sole discretion.
